STUDENT HEALTH INFORMATION REQUIRED BY AUGUST 3
Beginning July 1, families can log in to the Magnus Health Portal to complete or update required health information for each enrolled child. Please do not complete requirements before July 1, as they will need to be redone. All health requirements must be submitted by Monday, August 3.
- New families: Complete all required health forms and information (allow 10–15 minutes per child).
- Returning families: Review and update existing information as needed.
- Families with outstanding requirements will receive weekly reminders.
- If you also have a Valor student, please use your separate CHC Magnus login.
Accessing Magnus
The easiest way to access Magnus is through FACTS. Under Student, click Medical, and you will be directed to your child’s Magnus account. You may also log in directly at MagnusHealth.com using your CHC Magnus credentials.
Middle School Sports Physicals
Students participating in Middle School sports must have a current sports physical on file in Magnus before participation is allowed.
- Upload the completed physical form to Magnus.
- Exams are valid for one year from the date of the exam.
- Download the CHC MS Sports Exam Form 2026–2027.
Medication Drop-Off
Medications may be dropped off in the nurse’s clinic (Room 159) during Meet & Greet:
Tuesday, August 18
8:00 a.m.–3:00 p.m.
Medication Requirements
- Complete and upload all required medication forms in Magnus.
- Prescription medications and EpiPens must have a pharmacy-generated label. (Please provide two EpiPens if possible.)
- Over-the-counter medications must be labeled with the student’s first and last name and require a medication order form.
- Expired medications will not be accepted.
- Annual physician orders are required for all medications.
