Student Health Information Required by August 4

Beginning July 1, you will begin receiving Magnus Health notifications regarding required health information for your student(s). Please log in to the Magnus Health Portal to complete or update the required health information for each child enrolled at Cherry Hills Christian School. This information must be submitted no later than Monday, August 4.

  • New families: Enter all required health details.
  • Returning families: Review and update existing information as needed.
  • If there are any outstanding items in your Magnus account, you will receive weekly automated reminders until all requirements are fulfilled.
  • Once submitted, the CHC nurse will review and approve the information.

Note: Even if you’ve completed forms in the past, new and updated forms have been added. Please allow approximately 10–15 minutes per child to complete the updates.

All families with students in Preschool through Grade 8 must electronically sign the following forms within the Magnus Health Portal:

· Student Code of Conduct

· Parent in Partnership (Parent Code of Conduct)

· Parent-Student Health/Handbook Agreements

· Dress Code Guidelines: Preschool – Grade 8

· Middle School Missions & Offsite Field Trip Form

How to Access Magnus Health Portal

You can access Magnus in two ways:

  1. Directly via this link.
  2. Through your FACTS account.

Click on the Medical tab under your student’s profile to be directed to the Magnus portal.

Note: If you also have a student at Valor, you will need to use your separate login credentials for each school.

Middle School Sports Physical Form

If your child will participate in Middle School sports this school year:

· Download and print the Sports Health Exam Form.

· Have it completed and signed by your child’s physician.

· Upload the completed form and electronically sign the Sports Waiver in your Magnus Health account before the first day of school, Wednesday, August 13.

Medication Drop-Off Information

Medications will be accepted in the nurse’s clinic (Room 159) during Meet & Greet.

Medication Submission Guidelines:

If your child needs medication during the school day, including allergy or asthma treatments, please complete and upload the required Medication forms in the Magnus Health portal. All necessary forms can be found directly within the Magnus Health portal.

· EpiPens and prescription medications must have a pharmacy-generated label. Unlabeled prescriptions will not be accepted.

· Please bring two EpiPens, if possible (one for the clinic and one for the classroom).

· Over-the-counter medications must be labeled with your child’s first and last name.

· Check expiration dates—expired medications will not be accepted.

· Annual physician orders are required for all medications.
